Grove City Medical Center's Home Health Services is a Medicare certified home health agency located in Grove City, Pennsylvania.
We are committed to providing quality care to patients in their homes. Our staff of caring professionals works closely with the physicians to develop an interdisciplinary approach to care and administer quality care to each individual. We are dedicated to meeting the ever-changing health care needs of our community through the delivery of expert, compassionate and individualized care.
We believe that patients recuperate better, both physically and emotionally, when able to do so in the home within familiar surroundings. That's why we provide quality health care services and patient education to individuals in their homes.
Well informed patients who participate in their treatment are more likely to maintain a positive attitude toward recovery. Both the patient and the family are encouraged and taught to participate in the treatment program and become part of the health care team. Recovery in the home helps patients feel more comfortable and in control of their care.
Home Health Services
We provide the following in-home services:
Home Health Nurses: The nurses coordinate care, monitor and provide education about medications, management of illness, obtain lab work, provide wound care, manage and instruct about IV's, ostomies, catheters, and other special procedures. The nurse keeps the physician informed about the patient's progress.
Physical Therapy: The patient's capabilities are assessed and rehabilitative techniques are used to reach realistic goals. These techniques may include gait training, evaluation for equipment, home exercise programs, muscle strengthening, or instruction in the use of prosthetic devices.
Speech Pathology: Specific treatment programs are provided to assist with the rehabilitation of patients with speech, communication, or swallowing problems.
Occupational Therapy: The patient is assisted in regaining their independence. The treatment may include self-care training, splinting, therapeutic strengthening exercises, home management skills and adaptive advice training.
Social Workers: Services are provided to assist the patient and the family when social or economic factors interfere with the treatment or health maintenance or inhibit the patient in coping with disease-induced limitations. Services include: assessment, information and referral, casework, therapeutic counseling, and consultation.
Home Health Aides: Personal care services, such as bathing or hair and nail care, are provided at the direction of a registered nurse for those patients receiving nursing and/or therapy.
Dietary Consultation: This service is provided to nursing and therapy staff for their patients with nutrition related problems.
How to Obtain our Services
To obtain home health services, a patient must be under the care of a physician, and the patient must have great difficulty leaving the home for health care. Referrals are received from physicians, hospitals discharge planners, community agencies, family members, and patients themselves.

When a patient is in need of home health services, the physician and home care professionals establish a treatment plan for the patient. Home health staff will provide a periodic report to the physician allowing for reassessment of the treatment plan.
Services are available to all those eligible regardless of age, color, class, creed, sex, economic status, or handicap.
