Transitional Care Center
Transitional Care's nursing staff and therapists are committed to helping residents make the transition from hospital to home.
Transitional Care provides a level of care that exceeds that of a nursing home with the goal of preparing residents for discharge. While the majority of residents are admitted to TCC from GCMC's acute care unit, those who desire to have their therapy at GCMC may choose to transfer to TCC from other surgical centers or hospitals.
Conveniently located on the second floor of the hospital, this beautifully decorated unit provides intensive physical, occupational, and speech therapy as well as skilled nursing.
Staff Physical Therapists work with residents who have had joint replacements or other surgical procedures to regain strength and mobility. By the time residents are discharged they have already begun to experience the benefits of their surgery and therapy. Our physical therapists teach residents the necessary skills so that once they return home they are able to continue to heal and regain skills that are necessary for independent living.
Occupational Therapists at TCC design activities for patients that enable them to regain and build skills that are required for the completion of everyday tasks. For example, TCC has an "Activities of Daily Living" room that is furnished like a typical home kitchen. Residents practice activities that they will need to be able to perform upon returning home. These activities not only have medical value, but also help residents to enjoy their stay in TCC.
Speech therapists help patients to restore function in many areas such as speech and language, memory skills, thought organization, and swallowing abilities. In addition, voice therapy is available, including a program for residents with Parkinson's disease.
