Transitional care is exactly what the term suggests--care
provided for individuals transitioning from a major health
event back to their home, or to another care environment,
such as a long-term care facility. Those who have
undergone total joint replacements, such as a hip or a knee
benefit from the wide range of rehabilitative therapies
available. Along with physical therapy, the Transitional
Care Center offers speech and occupational therapy.
Individuals recovering from a stroke will progress with the
help of our multi-disciplinary approach to helping them
return to their activities of daily living. But, by far,
the most important aspect of Grove City Medical Center’s
Transitional Care Center is the homelike environment and the
caring, compassionate staff members. Private and
semi-private rooms are available to meet the needs of our
residents. We encourage residents to bring a few personal
items from home to create their own special place while they
stay with us.
We maintain an exceptionally high staffing level, which
allows our nurses and aides to provide highly individualized
care for residents. Speech Therapy

Helping coordinate all of the services we offer as well
as making plans for discharge is a dedicated social worker
who supports residents of our Transitional Care Center. Our
social worker is resourceful, innovative and extremely
qualified to help link residents with the appropriate
services they’ll need to make a worry-free transition.
Activities
Residents are provided with a wide range of activities
throughout each day, specially designed to entertain and
stimulate the senses. Residents may participate in as many
or as few as they wish, however, studies show that residents
who engage in activities progress far more quickly in their
rehabilitation than those who do not.
Beauty Services
A local hair stylist visits our Center weekly to provide
low-cost services to our residents.
Visits
Friends and family are welcome to visit residents. Pet
visits may also be arranged on an individual basis with
prior notice to our staff.
